PLEASE NOTE!! You must answer ALL questions for the form submission to complete.

  • Successful Submission: A new webpage will appear noting your submission was successful; you will also receive an email confirmation within 2-3 minutes including the zoom link to attend the event
  • Unsuccessful Submission: When the “submit” button is clicked, the form will just “clear” itself and you will be looking at a blank form. You will not see a “successful submission” message appear (or receive a confirmation email). Make sure you have answered ALL questions before pressing submit. If you see a blank form after pressing submit, you will unfortunately need to enter your information again.
    • Hint: Make sure you have not skipped the "checkbox" question regarding board type. And make sure that you have entered text into every field (it can be N/A if it does not apply to you).
